The type of document that is helpful for us to see are those which provide a lot of information around the additional support that your child or young person needs.<\/p>\n\n\n\n

If dated within the last 12 months, we can accept the following documents:<\/p>\n\n\n\n

A full copy of a report detailing the education support provided to help with your child or young persons special educational needs, such as a Statement of Special Educational Needs or an Individual Education Plan (IEP)<\/li>\n\n\n\n
A supporting letter from a Teacher, SEN staff member, Key worker or Portage team<\/li>\n\n\n\n
A therapeutical assessment report from a SALT, Neurologist, Audiologist, Clinical Psychologist or Paediatrician<\/li>\n\n\n\n
A supporting letter from a Doctor, Health Care worker or Therapist<\/li>\n<\/ul>\n\n\n\n
As part of your application, we will ask you to provide a document that names your child or young person. This is to ensure the person you are applying for lives with you. This could be a copy of an award notice for Disability Living Allowance (DLA) , Child Disabled Payment (CDP) , Personal Independence Payment (PIP) or Adult Disability Payment (ADP)<\/p>\n\n\n\n
However, we can not accept these as evidence of your child or young person’s eligibility, nor can we accept appointment letters or health and educational reports that are older than 12 months.<\/p>\n<\/body><\/html>\n","protected":false},"excerpt":{"rendered":"
